Reconnect is a free coordinated cycle social support program supporting people aged 16 years and over who are experiencing challenges with their mental health.
Our Peer Coaches work alongside you to build confidence strength and well-being and reconnect with your community. A formal mental health diagnosis is not required.
We support people in Perth North and Country WA.

Who can access Reconnect?
- If you are aged 16 years or older
- Live in Perth North or Country WA
- Have a mental health condition that impacts your daily life
- Are on waiting list for other services
- Are experiencing social isolation needs support to engage with community
- Require psychosocial support but are not eligible for NDIS cycle social funding
- Have recently been discharged from hospital

This Commonwealth Psychosocial Support Program (CPSP) is an evidence-informed, non-clinical approach to support you during the times when you need it most.

Established in Sydney in 1957, the founders were drawn together by their first-hand experience of mental ill-health. The wisdom they gained in helping each other overcome life’s challenges and move toward recovery was carefully recorded and this forms the basis of our GROW programs.
